Health Risk Assessment (HRA)

What is a Health Risk Assessment (HRA)?

The HRA is a 39-item online questionnaire and biometrics to assess your overall wellness, which is a great way to start any health program.

Once you complete the questionnaire and biometrics, you will receive a 7-page Personal Wellness Profile (PWP) and a 1-page body composition report.

Who needs to complete the HRA?

Participants 18 and older are required to complete the HRA and biometrics. Participants under the age of 18 are not required to complete the online questionnaire, but they are required to complete the biometrics.

How do I complete the HRA?

You will need to complete a User ID request form for each person interested in signing up for the program. We will send your log-in information (User ID and password) via email. Please complete the User ID request form at least 1 day before you attend an enrollment session.

Once you have your User ID, you need to log in to the Personal Wellness Profile website to complete your questionnaire. You can find that website here or by clicking on the yellow "Health Risk Assessment" button located at the bottom of this page.

When do I need to complete the HRA?

The HRA must be completed BEFORE you attend an enrollment session. If your online questionnaire is not completed when you attend an enrollment session, you will be required to complete it during the session. However, we cannot guarantee the availability of a computer to complete the online questionnaire at the enrollment session (due to high participant volumes). Questionnaires take approximately 10-15 minutes to complete, which can lead to longer wait times if you have not completed the questionnaire before attending the enrollment session.

What are Biometrics?

Biometrics include your:

  • Height
  • Weight
  • Blood Pressure
  • Body Composition

Is Lab Work Included?

No. If you would like lab work to be included in your HRA, you will need to submit a copy of your most recent lab work (preferably labs from the last 3 months). You can bring a hard copy to your sign-up session or you can have your physician fax a copy of your labs to us.

Who will have access to my information?

All of the information we receive from your HRA remains confidential. It is not given to any insurance companies or to your employer. After the end of the enrollment period, you will receive your report via email. You can share your reports with any friends, family members or co-workers. Calvert Memorial Hospital does not share the information from your report with anyone.
