Comprehensive Health Management
Our mission at the CMH KeepWell Center is to promote health, prevent disease and enhance quality of life. KeepWell@Work provides employers with the tools they need to effectively manage the health of their employees. We provide full-service capability to companies who want to integrate these features into their benefit plan design.
Health Risk Assessment (HRA) - a 39-item questionnaire with biometric testing (height, weight, body composition, blood pressure and cholesterol testing). Each participant receives a seven-page personal wellness report. It helps employees evaluate their current lifestyle, identify health risks and target areas for improvement. This information can be valuable in designing a customized wellness program for your worksite.
Health/Wellness Coaching - According to the Wellness Council of America, a $1 investment in wellness yields a $3 return on investment (ROI). Health and wellness coaching is a cost-effective strategy for health plans, employers and providers to contain the rising cost of chronic conditions. Most referrals are from an HRA and are delivered in person, via e-mail or by phone. Health coaching focuses on applying interventions, appropriate to the employee's current level of readiness, while encouraging them to the next level.
Disease Management - We work with the employee's primary care physician to help him or her manage their disease and optimize their wellness potential.
*Medical Management - Our partner provides the web-based technology to help identify the employees who are most at risk and would benefit most from a medical management intervention. These tools include:
