How To Start

These instructions will help you get started with the KeepWell@Work program.  It's as easy as 1-2-3!

Step 1:

Complete the User ID Request form. You will receive an email containing your user ID and link to complete the online questionnaire. You will receive your unique log-in information by email within 1-2 BUSINESS days. Your information will be sent to the email address you provide on the User ID Request Form.

Step 2:

Log on to the WellSuite Personal Wellness Profile using the link you received after completing the User ID Request form.

Once you have your user ID and password, click here to access the Online Questionnaire Website or click the yellow "Health Risk Assessment" button located in the brown box at the bottom of this page.

Step 3:

Attend one of the enrollment sessions (see Schedule). You will complete the sign up process at the enrollment session.
